Everyone at the machine can say what the guard is for. They can say it in the briefing, in the audit and on the form. Then a part jams, the shift is behind, and a hand goes in while the spindle is still turning. Nobody forgot the rule. The hand simply moved first.
That second is not rare. The DGUV counts 150,908 reported accidents a year in this cluster, most of them on an ordinary shift with an experienced person at the machine. What changes it is not another briefing but having been in that second before, often enough that the hand stops on its own.
Each training is one task at a machine or a bench, where a hand and a moving part end up in the same place. The time is a single run, from the first word to the last, and it is what tells you whether it fits into a shift.
